Getting your DOT physical exam has never been easier or more convenient for your busy work schedule.
In today’s busy world, we understand time is valuable. Our nationally registered certified medical examiners, Dr. Rourke and Dr. Spaulding, will do everything possible to keep you on the road legally and in a timely manner. Medical Cards may be provided same day, pending results. Here’s what to expect during your DOT physical exam:
STEP 1: Schedule your DOT Physical appointment
Call (814) 730-2666 to schedule an appointment prior to your visit to reduce your wait time and ensure a DOT accredited examiner is currently available at the office to provide your DOT physical. Our office is located in Edinboro at 106 Waterford St, beside The Flip Cafe.
STEP 2: Get your DOT physical
We will perform an examination, including:
- Health history review
- A thorough physical exam
- A urine test
STEP 3:
We’ll complete the Medical Examination Report for Commercial Driver Fitness Determination form and its supporting documentation, including DOT physical forms.
You must pass all DOT physical requirements in order to be cleared for your CDL medical card.
Keep the following in mind before your DOT physical visit:
- To speed up your visit please fill out and print the driver’s portion of the Medical Examination Report prior to your visit.
- Here are some recommendations on what you should bring to qualify for your commercial driver’s license medical exam:
Current State Issued Driver’s license
Commerical Driver’s License ( CDL ) if applicable
Previous DOT Medical Card if applicable
Any corrective lens or hearing aids.
Medication List
Current treatments such as readings from a CPAP machine if you have sleep apnea, or blood sugar readings if you have diabetes.
Any medical waivers or releases from your primary care physician.
